Monthly Prize Draw Terms and Conditions
Eligibility: The Monthly Prize Draw ("the Draw") is open to all customers who leave a review on Trustpilot for MyHealthChecked ("the Company"). Participants must be at least 18 years of age and residents of the United Kingdom.
Entry: To enter the Draw, participants must leave a single review on Trustpilot regarding their experience with the Company. Only one entry per individual per month is allowed. Multiple entries will not be counted.
Draw Process: As of 05/04/2024, by the 29th of each month, one winner will be randomly selected from the pool of eligible entries. The Draw will be conducted fairly and impartially by representatives of the Company.
Winner Notification: The selected winner will be asked to share their contact information via the email address associated with their Trustpilot review within 7 days of the Draw. The winner must respond to the notification within 14 days to claim their prize.
Prize: The winner of the Monthly Prize Draw will receive a MyHealthChecked test of their choice, with a value of up to £85. The prize cannot be exchanged for cash or any other alternative.
Forfeiture: If the selected winner fails to respond within 14 days of notification, they will forfeit their prize, and another winner will be randomly selected from the remaining eligible entries.
Data Protection: Any personal information provided by participants will be used solely for the purpose of administering the Draw and will not be shared with third parties without consent, except as required by law.
Modification or Termination: The Company reserves the right to modify, suspend, or terminate the Monthly Prize Draw at any time without prior notice, for any reason.
Governing Law: These Terms and Conditions shall be governed by and construed in accordance with the laws of England and Wales.
Acceptance of Terms: Participation in the Draw constitutes acceptance of these Terms and Conditions.
